New Delhi: Worldwide public health is seriously threatened by cholera, a highly contagious watery disease brought on by the Vibrio cholerae bacteria. Understanding the relationships between water and sanitation conditions and the dynamics of cholera transmission is crucial for developing successful intervention methods.
The World Health Organisation (WHO) estimates that cholera kills between 21,000 and 143,000 people worldwide each year and infects between 1.3 and 4.0 million people. Low- and middle-income nations are disproportionately affected by the cholera pandemic, especially those with poor access to sanitary facilities and clean water.
Polluted water, insufficient sanitation infrastructure
One of the main sources of infection is contaminated water, which is frequently contaminated by faeces that contain Vibrio cholerae. This emphasises how crucial it is to guarantee that water sources are pure and uncontaminated.
Dr Sanjay Khanna, Director and HOD, Gastroenterology, Fortis Hospital, Shalimar Bagh, said, “Contaminated water and inadequate sanitation facilities have a crucial part in serving as the main channels of transmission for cholera. A substantial risk of infection exists from contaminated water sources, which are frequently loaded with Vibrio cholerae from faecal matter. Infection can result from drinking such water or using it for routine tasks like cooking, cleaning, and bathing. Inadequate sanitation infrastructure also makes the issue worse by allowing human waste to contaminate water sources. The problem is made worse by behaviours like open defecation and improper sewage disposal, especially in areas where there is a shortage of clean water and sanitary facilities. It’s critical to address these environmental elements in order to prevent cholera outbreaks and reduce the disease burden, but it’s also imperative to emphasise the need of maintaining proper hand and personal cleanliness.”
Cholera germs can flourish in water sources due to the combination of rising temperatures and more rainfall. The risk of cholera transmission can increase due to sewage contamination of drinking water caused by heavy rains. The disease can spread even more during floods due to the environment’s exposure to contaminated water and the absence of adequate sanitary facilities.
Dr Khanna added, “Seasonal differences’ impact on the transmission of cholera, specifically on temperature and precipitation. The perfect environment for cholera germs to proliferate and reproduce in water sources is one of warmth and precipitation. Sewage contamination of drinking water by heavy rainfall can worsen the condition and raise the danger of transmission. Floods exacerbate the issue even further by encouraging the use of contaminated water or open defecation, which helps the disease spread. While outbreaks of cholera may happen more frequently during the warmer months in milder climates, the disease may remain year-round in hotter locales. It is essential to comprehend these periodic trends in order to effectively prevent cholera epidemics. Actions including keeping an eye on the illness, enhancing access to clean water and sanitary facilities, and teaching people about preventive measures against cholera.”
Preventing cholera epidemics and mitigating the disease’s impact on impacted populations requires addressing environmental issues such as tainted water supplies, insufficient sanitation infrastructure, and inadequate hygiene habits. This entails expanding access to clean water, putting in place suitable infrastructure for sanitation, and encouraging hygienic habits.
Rural people are more susceptible to cholera outbreaks because they frequently lack access to basic sanitation facilities and safe, dependable supplies of clean water. Controlling cholera epidemics efficiently might be difficult in isolated rural regions due to limited access to healthcare services and a possible lack of understanding about good hygiene practices.
Dr Manjusha Agarwal, Senior Consultant, Internal Medicine, Gleneagles Hospitals, Parel Mumbai, said, “The different but equally important problems that people in rural and urban areas encounter when there is a cholera outbreak. Densely populated metropolitan environments, where access to clean water and good sanitation may be limited, can facilitate the spread of waterborne diseases such as cholera. The risk of epidemics in these densely populated places is increased by inadequate sewage systems and contaminated drinking water. On the other hand, poor sanitation facilities and inappropriate supplies of clean water present unique difficulties for rural populations, which further contribute to the spread of cholera. Effective cholera control initiatives are further hampered by the lack of knowledge about good hygiene practices and the dearth of healthcare facilities in rural regions. Customised interventions that increase awareness and access to sanitary facilities and clean water are necessary to address these complicated concerns.”
Curbing the spread of cholera through sanitation tactics
Boiling water eliminates dangerous bacteria and pathogens, including cholera-causing ones, and is a straightforward yet efficient way to make water safe to consume. This strategy is especially helpful in places with inconsistent or restricted access to clean, treated water. Boiling water is a home task that can benefit communities even in resource-constrained environments.
A community’s ability to contain the spread of cholera can be aided by educating people about safe food handling techniques and appropriate sanitary etiquette. Communities can reduce the danger of cholera outbreaks and transmission by increasing awareness of the need of keeping clean and hygienic surroundings.
Dr Manish Kumar Tomar, Consultant Gastroenterology, Yatharth Super Speciality Hospital, Noida Extension, added, “Boiling water, encouraging hand hygiene, and avoiding cross-contamination are important strategies for improving community access to clean water and sanitary facilities, particularly in areas where cholera outbreaks are likely to occur. In places without access to treated sources, boiling water is an effective and easy way to get rid of dangerous germs and pathogens, making drinking water safe. Simultaneously, encouraging good hand hygiene—most notably, frequent handwashing with soap and water—decreases the likelihood of cholera spreading from person to person and stops contaminated water from being consumed. In addition, community efforts to stop cholera outbreaks are strengthened by highlighting person-to-person infection prevention behaviours such as safe food handling and appropriate sanitation procedures. By combining these strategies with more comprehensive plans to upgrade the infrastructure for sanitary facilities.”
Better sanitation facilities, such as appropriate sewage disposal systems and sanitation infrastructure, lessen the chance of cholera outbreaks by preventing the contamination of water sources with cholera-causing faecal matter. Improved sanitary infrastructure also helps to reduce the spread of other pathogen-caused watery ailments, such as viruses, bacteria, and parasites. The spread of illnesses including dysentery, typhoid fever, and diarrhoea is stopped by good sanitation.
Dr Tomar mentioned, “Enhancing sanitary facilities and raising the quality of the water are important activities that not only help prevent cholera but also advance other public health goals. Communities can successfully stop the spread of cholera and other watery diseases brought on by bacteria, viruses, and parasites by enhancing sanitation infrastructure, such as adequate toilet facilities and sewage disposal systems. The health of the populace benefits greatly from programmes aimed at enhancing water quality, such as purifying water sources to eliminate impurities and guaranteeing that everyone has access to clean drinking water. In addition to being essential for maintaining personal cleanliness, cholera prevention, and food preparation, access to clean water also lowers the risk of other waterborne illnesses like dysentery, typhoid fever, and diarrhoea.”
To stop the spread of cholera, people must have access to clean drinking water and better sanitary facilities. Still, billions of people around the world do not have access to these fundamental needs. About 2.2 billion people and 4.2 billion people, respectively, lack access to safely managed drinking water and sanitation services, according to The United Nations International Children’s Emergency Fund (UNICEF) and WHO.
Water, sanitation, and hygiene conditions are directly related to the dynamics of cholera transmission. A comprehensive strategy that includes infrastructure development, behaviour change treatments, surveillance systems, and coordinated intervention techniques is needed to address the underlying causes of cholera.
Through the utilisation of precise data and evidence-based strategies, we may successfully tackle cholera and enhance public health results worldwide. The key to controlling cholera and eventually eradicating this preventable disease is to invest in infrastructure related to water and sanitation, encourage multisectoral collaboration, increase surveillance capabilities, and promote hygiene education.
